To Appeal For A Ban
1. Your ingame name and STEAM ID
2. Server (if known)
3. time &/or date (if known)
4. Reason For Ban
5. Commitment After Ban is taken off (if it is taken off)
6. once your bans destiny is chosen do not try again the poll will be deleted
Follow these and you have a chance of removing your ban
